Creatine got typecast early. It sits on the shelf between the pre-workouts and the mass gainers, sold to guys chasing a bigger bench, and that marketing has quietly kept one of the most researched compounds in sports science away from the people with the most to gain from it.
Because creatine after 40 changes the reason you take it. The muscle benefit is still there. It just stops being the headline.
First, What Creatine Actually Is
This part gets skipped and it’s the part that makes everything else make sense.
Creatine is not a stimulant, a hormone, or a steroid. It’s a compound your body already makes from amino acids, and you also get it from red meat and fish. Roughly 95% of it is stored in your skeletal muscle as phosphocreatine, with a small but meaningful amount in your brain.
Its job is energy. Specifically, it helps regenerate ATP, the molecule your cells burn for everything they do. During short, hard efforts, a heavy set, a sprint, a max effort, your ATP drains fast and phosphocreatine is what refills the tank. More phosphocreatine stored means more repeatable output before you fall apart.
Supplementing does one thing: it tops up the stores you already have. Most people walk around partially saturated. Creatine fills the rest.
Hold onto that mechanism, because it’s the reason the brain research isn’t as strange as it sounds.
What Starts Going Wrong After 40
Three systems begin sliding, and they don’t announce themselves.
Muscle. From your mid 30s onward you lose roughly 1% of muscle mass per year. It accelerates through your 50s and 60s. What matters more than the number is which muscle you lose first: type II fibres, the fast, powerful ones. That’s why aging shows up as a loss of power before a loss of size. Getting out of a low chair. Catching yourself on a stumble. Hauling something up a flight of stairs. Those are all power tasks, and they degrade first.
Bone. Peak bone density arrives in your late 20s and declines from there, faster for women after menopause, but men are not exempt. Combine falling bone density with falling power and you get the actual risk: a fall you can’t catch yourself from, landing on a skeleton that’s less able to absorb it.
Brain. Processing speed, working memory, and reaction time all decline measurably with age, and it starts earlier than most people want to hear.
Creatine has published research touching all three. The strength of that evidence varies a lot depending on which one, so let’s be honest about which is which.
The Muscle Evidence: Strong
This is the most settled part of the picture.
Dr. Darren Candow at the University of Regina has spent his career studying creatine specifically in older adults, and his work is the backbone of this conversation. The consistent finding across studies: creatine combined with resistance training produces greater gains in lean mass and strength than resistance training alone in older adults.
Note the second half of that sentence. Creatine is not a replacement for training. It’s a multiplier on training you’re already doing. Take it and sit on the couch and you get the water weight and not much else.
The interesting wrinkle is that older adults appear to respond at least as well as younger ones, and possibly better. One likely reason is baseline. Muscle creatine stores tend to be lower in older adults, partly because they often eat less meat and less protein overall. Lower starting stores means more room to fill, which means a bigger jump when you fill them.
The group that supposedly isn’t the target market is the group with the most headroom.
The Brain Evidence: Promising and Growing
Here’s where it gets interesting.
A meta-analysis published in Nutrition Reviews found creatine supplementation improved memory performance in healthy adults, with the strongest effects appearing in people over 66.
Go back to the mechanism. Your brain is metabolically expensive, it runs on the same ATP system your muscles do, and it stores phosphocreatine too. When that energy system is under strain, and aging, poor sleep, and stress all strain it, extra fuel in the tank appears to help.
Two honest caveats. First, this research is newer and smaller than the muscle work, so treat it as promising rather than proven. Second, creatine crosses into the brain more slowly and less easily than it moves into muscle, which is why some researchers, Candow included, have raised the question of whether brain effects need higher doses or longer timelines than the standard muscle protocol. That question isn’t settled. Worth knowing before someone sells you a “brain dose.”
The Bone Evidence: Weakest of the Three
We’re not going to oversell this one.
Some studies show potential benefit for bone health, particularly in postmenopausal women and older men, usually when creatine is paired with resistance training rather than taken on its own. But results are inconsistent across studies, and Candow’s own recent review stays cautious.
File it under “possible bonus,” not “reason to start.”
How to Actually Take It
Form. Creatine monohydrate. That’s it. It’s the form used in the overwhelming majority of the research, it’s the cheapest, and no fancier version with a longer name and a higher price has convincingly beaten it. Ignore the marketing.
Dose. 3 to 5 grams daily. For older adults, sit at the top of that range. 5 grams.
Loading. Optional and honestly unnecessary for most people. A loading phase, roughly 20 grams a day split into four doses for five to seven days, saturates your muscles faster. Skipping it and taking 5 grams daily gets you to the same saturation in about three to four weeks. Loading also causes more stomach upset. If you’re not on a deadline, skip it.
Timing. Barely matters. Post workout shows a slight edge in some studies but the difference is small enough to ignore. Take it when you’ll remember, in whatever you’re already drinking.
Consistency. This is the one that actually matters. Creatine works by saturating tissue over time, not by producing an effect in the next twenty minutes. It’s closer to a vitamin than a pre workout. Miss days regularly and you slowly drain the saturation you spent weeks building. Every day, training days and rest days alike.
What to expect, and when. Nothing on day one. In the first week or two you may notice slightly better output on your last couple of reps, and slightly less of a drop off across sets. That’s the phosphocreatine system doing its job. The meaningful stuff, more quality reps compounding into more muscle and strength over months, is downstream of that.
Three Things People Get Wrong
“It’ll make me bloated.” Creatine pulls water into your muscle cells, not under your skin. The scale may move a pound or two early on. That’s intracellular water inside the muscle, which is where you want it, and it isn’t the puffy look people are picturing.
“It’s hard on your kidneys.” This is the big one, and it’s a misreading of a lab result. Creatine metabolism raises a blood marker called creatinine, which is the same marker doctors use to screen kidney function. So a routine panel can look alarming when nothing is actually wrong. Decades of research show creatine monohydrate is safe at standard doses in healthy people. Just tell your doctor you’re supplementing so nobody chases a phantom.
The real exception: if you have existing kidney disease, talk to your doctor before you start. That’s not a disclaimer, that’s the actual rule.
“It didn’t do anything for me.” Some people are genuine low responders, usually because their baseline stores were already high, often from eating a lot of red meat and fish. If your tank is nearly full, filling it changes less. Flip side: if you eat little or no meat, your baseline is likely lower than average and you may notice more than your steak eating training partner does.
The Bottom Line
Creatine monohydrate. 5 grams. Every day. Alongside training that actually challenges you, because creatine amplifies work, it doesn’t replace it.
It’s the most studied supplement in existence, it costs pennies a serving, and after 40 the reasons to take it stack up well past what it does for your lifts. Muscle you’re actively losing. A brain running on the same energy system. Possibly your bones along for the ride.
Not bad for something the industry still markets to 25 year olds.
